SPRING REGISTRATION

WHAT:   Preschool (4K) and Kindergarten (5K)

Registration for the school term 2011 - 2012

WHO:    Children who are four years old on/before

September 1, 2011 will be eligible for the

4K preschool program:  pending screening

Children who are five years old on/before

September 1, 2011 MUST be enrolled in a 5K program

WHEN:   Tuesday, April 12th         8:00 a.m. - 12:00 noon

Tuesday, April 12th          3:30 p.m. - 6:30 p.m.

Wednesday, April 13th     8:00 a.m. – 2:00 p.m.

Thursday, April 14^th          8:00 a.m. – 12:00 noon

Thursday, April 14^th          3:30 p. m. –6:30 p. m.

WHERE:  Richard Carroll Primary School, 153 McMillan Street

Enter the school’s main entrance and then follow the signs to

the Reading First Book Room.

PARENTS MUST HAVE THESE DOCUMENTS IN ORDER TO REGISTER.

• Birth Certificate
• Social Security Card
• Proof of Residency (Utility Bill/Phone Bill)
• Medicaid Card (if applicable)
• Guardianship Document (if applicable)
• Completed South Carolina Immunization Record (DHEC Form 1148)                                                      (If your child is not 4 years old at the time of registration DHEC Form 1148 is still required, however it does not have to be completed/ All 5 years old must have a completed DHEC Form 1148)
